This contest between two well-matched, mid-field teams featured strong proponents on each side scoring points on which trend is doing more to meet the healthcare needs of modern patients. Is it Consumerization of Healthcare Demand (4), which is opening more direct channels for patients to care for themselves, or is it Product Specialization Accelerates (5), the effort of health plans to offer more specialized products tailored to specific populations? In a second half rally, consumers pull away from product specialization and advance to the next round.

Either way, voters have decided that the future is direct contracting, as employers are increasingly going direct for employee care and bypassing traditional insurance channels to gain greater cost control, transparency, and access. In a tremendous upset, Acceleration of Direct Contracting leaves VBC wondering what just happened.

Experts were split, but voters were decisive, and Price Transparency Enforcement moves on to the Round of 16.

Telehealth at a Crossroads Crosses Out Interoperability Gains Ground

In this “Battle of the E-Health,” Interoperability Gains Ground (6) came in as another fan favorite, with high hope of 2026 being the year interoperability goes deep into the tournament – and further into portable electronic health records. Specialists Under the Value Lens (7) – although elated just to be in the tournament – had no weapons in their arsenal to match the muscle of Drug Pricing Shakeup (2). A smorgasbord of drug pricing reform policies in 2026 are reshaping how plans structure formularies, how patients experience costs, and how providers and manufacturers manage revenue.
